Hospital care

Specialist and Hospital Care

Should you need referring to a Specialist for your care you will be given a choice as to where you would like that appointment to be and which Consultant/Specialist you would prefer.
The Surgery is able to organise the appointment electronically whilst you are with the Doctor – this is known as ‘choose and book’.

However if you would prefer to have some time to think before deciding where and when to have treatment, you will be offered the option of calling the surgery at a later time and we will be able to book your appointment then.

Health checks and appointments for the over 75’s

NHS Health Checks
We are currently sending out an invitation for a Health Check to everyone between the ages of 40-74years of age on a 5 yearly rolling programme.
The Health Check is to assess your risk of developing heart disease, stroke, kidney disease or diabetes.
By identifying risk factors before they have created symptoms, we can take early action that will improve your health and prevent the onset of these conditions. There is hard evidence to show that ‘prevention IS better than cure’.
The Health Check will take approximately 30 minutes. You will be asked a series of questions about your current health and famaily history and measurements taken of your blood pressure, height and weight. a blood test will be taken to measure your cholesterol and blood sugar.
You will be advised about your health risks dependent on your results and what you can do to stay healthy.

Over 75’s checks
Health checks are also being offered to our patients over 75 years of age. These can be a stand alone health check or they can be part of a patient’s chronic disease review with the practice nurse if they also have a chronic disease such as asthma, COPD (chronic obstructive pulmonary disease), heart problems etc. The over 75 health check will take 30 minutes so please ask for this length of time with the nurse when booking you appointment.

Women’s Health

Cervical Smears
It is recommended that all women between the ages of 25 – 65 years of age have regular smear tests. The Primary Care Trust – on our behalf – operate a ‘call and recall system’ to remind you of this – so you will get a reminder letter from them every 3 years until the age of 50 and then every 5 years until you are 65 years of age.
You can make an appointment with the Practice Nurse for a cervical smear – the ‘ideal/best’ time for a cervical smear is mid way through your menstrual cycle. Do not make an appointment for a smear if you have your monthly period.

Please bring a urine sample with you for this appointment.

HRT
Our GPs undertakes the HRT checks and can prescribe your repeat prescriptions.

Family Planning
Family planning services are available at Slaidburn Health Centre either with the Doctor or Practice Nurse.
We are currently unable to offer either Coil or Contraceptive Implant Fittings.

If you prefer there is a clinic at Clitheroe Hospital on Wednesday  at evening 6.30pm-9.00pm. Please telephone Clitheroe Hospital on 01200 420660 to make an appointment there.

Travel Immunisations

Our Practice Nurse offers a very comprehensive travel health service specifically related to your destination and the pursuits you may be undertaking whilst travelling. You may also require travel vaccinations.

Please either print out the travel questionaire below, complete and post, deliver to the surgery or complete the form electronically AT LEAST 7 weeks BEFORE travel.

The Practice Nurse will then decide which vaccinations you require after having checked your medical records and the requirements for the country/ies you are visiting. Please ring the Health Centre one week after submitting the completed travel questionaire to see if you need any travel vaccinations.

Most travel vaccinations are ordered on a private prescription so there may be a charge over and above the normal prescription charge – this is because travel vaccination is not included in the services provided by the NHS and therefore is a chargeable service.

Click here to download our travel questionaire as as a PDF

and email the completed form to: teamslaidburn@nhs.net

Childhood Immunisations

The childhood immunisations are carried out by our Practice Nurse when she is on duty at a time convenient to you.

It is necessary for a consent form to be signed by a parent of the child for EACH immunisation. If someone other than the parent is bringing/accompanying the child for the immunisation please contact the surgery to arrange signing of the consent form prior to the immunisation appointment.

Here’s a checklist of the vaccines that are routinely offered to everyone in the UK for free on the NHS, and the ages at which they should ideally be given.
If you’re not sure whether you or your child have had all your routine vaccinations, ask your GP or practice nurse to find out for you. It may be possible to “catch up” later in life.
Try to make sure you or your child have vaccinations delivered on time to ensure protection. If you’re going to be away from the GP surgery when a vaccination is due, talk to your doctor. It may be possible to arrange for vaccination at a different location.
